Portfolio Evaluation Form

The following form is provided as a sample for self and peer evaluations of the Immersion experience and portfolio. It may be copied and used to hand in to instructors at specified points in the semester.

  • Use the associated assessment rubric to assign one of the three levels (E = Exceeds expectations,
    M = Meets Expectations & B = Below Expectations) to the first column for self evaluation.
  • Fill in the initials of the colleagues on your team and assign them levels for each of the six portfolio evaluation elements.
  • In addition, add any commentary notes on your own or other's performance in the space below each evaluation element.
